(d) Reporting of Other Non-Compliance. The permittee shall report all other instances of noncompliance not otherwise reported at the time monitoring reports required in Part II, Section D of this permit are submitted. The reports shall contain the informa- tion listed in Permit Condition 12 (c) (2) above. (e) Other Information. When the permittee becomes aware that he failed to submit any relevant facts in the permit application or submitted incorrect informa- tion in a permit application or in any report to the Director, the permittee shall submit such facts or information in writing within 10 days. (f) Report on Permit Review. Within thirty (30) days of receipt of this permit, the permittee shall report to the Director that he has read and is personally familiar with all terms and conditions of this per- mit. F. COMMENCING INJECTION. An operator may not commence injection until: 1. The information specified in Part III of this permit has been submitted to the Director for approval, 2 . Mechanical integrity of the well has been demonstrated in accordance with Part I, Section H, of this permit, 3 . Construction is complete and the permittee has submitted to the Director, by Certified Mail with return receipt requested, a notice of completion of construction using EPA Form 7520-9, and the Director has inspected or other- wise reviewed the new injection well and finds it is in compliance with the conditions of this permit, and 4 . The operator has received written authorization, from the Director, to inject. Prior to commencement of initial injection, all injection-related Federal, State, and Lo- cal permits and approvals must be obtained by the per- mittee. The permittee must be in compliance with all injection-related Federal, State, and local laws. G. PLUGGING AND ABANDONMENT 1. Notice of Plugging and Abandonment The permittee shall notify the Director no later than 45 days before abandonment of the well . 10
