TRUCKED NON-HAZARDOUS WASTE EBMUD DISPOSAL PERMIT -Terms and Conditions TRUCKED NON-IIAZARDOUS WASTE DISPOSAL Copperford,LLC Permit No. 2241 12470 Locke Road,Bldg. 100 Permit Issued—07-12-2002 Lockeford,CA 95237 Permit Expiration—06-30-2006 GENERAL CONDITIONS • This Trucked Non-Hazardous Waste Disposal Permit(Permit)is issued to Copperford,LLC for the disposal of trucked non- hazardous waste to the EBMUD Main Wastewater Treatment Plant(WWTP). • All trucks must be listed in the Liquid Waste Acceptance Agreement of this Permit and display a valid WINTP Entry Pass in order to enter the WWTP. • Copperford,LLC shall maintain a valid county permit(s) for hauling liquid waste. • Copperford,LLC shall supply hoses and other equipment necessary to properly dispose of trucked non-hazardous waste. • Copperford,LLC shall not dispose any hazardous waste,as defined in Section 251 17 of the California Health and Safety Code,to EBMUD Wastewater Facilities. COMPLIANCE REQUIREMENTS • Copperford, LLC shall not discharge any hauled waste from a source that has not been identified in the Liquid Waste Acceptance Agreement. • Copperford,LLC shall comply with the requirements outlined in the EBMUD Certificate of Public Liability Insurance. • Copperford,LLC shall comply with the attached EBMUD Procedures for the Delivery of Trucked Non-Hazardous Waste. • Copperford,LLC shall dispose all trucked non-hazardous waste at the disposal locations designated on the attached EBMUD Trucked Non-hazardous Waste Map of Discharge Locations. REPORTING REQUIREMENTS • Copperford,LLC shall provide EBMUD,upon request,records,pumping logs,manifests or analytical results pertaining to the disposal of trucked non-hazardous wastes authorized under this Permit. • Copperford,LLC shall immediately report to the EBMUD Source Control Division any changes to the truck information, equipment or operations,or quality or volume of trucked non-hazardous waste which deviate from the Terms and Conditions under which this Permit was granted. WASTEWATER DISCHARGE LIMITS Copperford,LLC shall not dispose of any waste that may interfere with Treatment Plant Operations. ENFORCEMENT AND PENALTIES Copperford,LLC is subject to enforcement remedies and penalties in accordance with EBMUD Ordinance No.311,Title VI. PERMIT FEES AND DISPOSAL CHARGES Copperford,LLC fees and charges arc due when billed by the District. Fees and charges shall be billed as agreed upon in the Liquid Waste Acceptance Agreements completed for each source delivered to the WWTP. Payment of the following fees and charges are due when billed by the District. Permit fee $25.00 AUTHORIZATION Copperford,LLC is hereby authorized to dispose trucked non-hazardous waste subject to Copperford, LLC's compliance with EBMUD Ordinance No.311 and the Terms and Conditions of this Permit.
