STAMPEDE ROAD LODI, CA 95240 fRMIT/SERVICES Cf- 4 I PA-0600-243 v / 12-3 aa'3.>r The following Sections are revised to read as follows: /3.6 Discussion of General Area Septic Tank Repair History: Repair history of the septic tanks in this area reveals that the leach lines and seepage pits have resulted in consistent satisfactory performance. Septic system repair has been limited to the addition of a single leach line to the existing systems and has occurred at only two hof the residences in the area. ✓4.3 Description of location, type and construction of well and location where water sample(s) were obtained: There is a single diesel motor powered agricultural well on the property which is located at the north end of the proposed Parcel 1 . Water samples were taken from a spigot on the outlet pipe of the well. The Environmental Health Department has previously indicated that there is no record of this well and therefore very little is known about well construction. This well will not be used as as domestic water source. ✓4.4 Description of constituents being tested for in the water sample: Water samples were tested for nitrates and DBCP. See Appendix for results. x/4.5 Description of how sample was obtained, handled, stored, and transported to the lab: The water samples were taken from a spigot near the well by Jake Handel of Dillon & Murphy and driven directly to Precision Enviro-Tech for analysis. The samples were taken and handled in accordance with Precision Enviro-Tech requirements. Test results can be found in the Appendix. ✓' 4.6 Complete Chain of Custody document, including printed and signed name of thirdparty sampler, condition of sample and date and time the sample was submitted to the lab. For Laboratory samplers, complete laboratory report to include the above noted information: The complete chain of custody document is located in the Appendix. Appendix The Appendix is revised to include the attached water test results and chain of custody document.
