Hazelton Avenue <br /> Stockton, CA 95205 <br /> Dear Mr. Rubianes: <br /> Caltrans has reviewed the above Application (PA-02-465) for a private right-of-way for <br /> six lots in an antiquated subdivision. The project site is located on the south side of State <br /> Route (SR) 12, 2300 feet west of Tretheway Road, east of Lodi (APN 051-140-01). <br /> Caltrans has the following comments: <br /> 1. Driveway connection to SR 12 shall conform to Caltrans standards. <br /> 2. An Encroachment Permit will be required from Caltrans. The environmental <br /> document prepared for the project that includes Caltrans right-of-way must be <br /> submitted with the Encroachment Permit application. Caltrans also requires <br /> documentation that cultural (archaeological), biological, and hazardous waste surveys <br /> have been conducted within Caltrans right-of-way. <br /> 3. For cultural surveys a recent record search from the information center and an <br /> Archaeological Survey Report are required. <br /> 4. A Natural Environment Study report shall be written documenting the results of <br /> biological surveys and the record search from the California Department of Fish and <br /> Game Natural Diversity Database. A qualified biologist should conduct surveys at the <br /> appropriate time of year to determine if listed plant or animal species or wetlands <br /> occur 1n the area. Surveys should meet the protocol standards of the U.S. Fish and <br /> Wildlife Service and the California Department of Fish and Game. <br /> 5. if right-of-way is being dedicated to Caltrans, the applicant is required to submit a <br /> copy of Attachment A, confirming that the land to be dedicated to Caltrans is free of <br /> hazardous waste. Even if right-of-way is not being dedicated to Caltrans it is a good <br /> practice to conduct a record search to obtain known hazardous waste locations. <br /> 6. It is highly recommended to contact the Native American Heritage Commission <br /> (NAHC).
