All monitoring and sampling results shall be reported to the City. The report should indicate the nature and concentration of all pollutants in the effluent which are regulated by the limits set forth in mart 1 A. Reports shall include a copy of all laboratory analysis reports, a summary of any violations of provisions in this permit, and a summary of other monitoring activities (such as pH and flow monitoring). 2. All wastewater user reports must be signed by an authorized representative of the user and contain the following j certification statement: "I certify under penalty of law that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fine and imprisonment for knowing violations." 3. If the permittee monitors any pollutant more frequently than required by this permit, using test procedures approved under 40 CFR Part 136 or as specified in this permit, the results of such monitoring shall be included in the calculations and results shall be reported in the monitoring report and submitted to the City of Lodi, 4.All monitoring reports are due 30 days after the conclusion of monitoring or receipt of the laboratory analysis report and shall be sent to the Water Services Manager at 1331 S. Ham Lane, Lodi,CA 95242, 5.All records must remain available for a period of at least three years per 40 CFR 403.12(0). B.Accidental Discharge Report 1. The permittee shall notify the Water Services Manager or White Slough Water Pollution Control Facilities (WSWPCF) personnel immediately upon the occurrence of an accidental discharge of substances prohibited by City of Lodi Municipal Code, Chapter 13.12. During normal business hours the City of Lodi should be notified by telephone i at 333-6740. At all other times, the City of Lodi should be notified by telephone at either 333-6749 or 368-5735. The j notification shall include location of discharge, date and time thereof, type of waste, including approximate concentration and volume, and corrective actions taken. Within ten days following anaccidental discharge, the permittee shall submit to the City of Lodi a detailed written report.The report shall specify the following: a. Description and cause of the upset, slug or accidental discharge and.the impact on the permittee's compliance status. The description should also include location of discharge,type,concentration and volume of waste. b. Duration of noncompliance, including exact dates and times of noncompliance, and if the noncompliance continues,the time by which compliance is reasonably expected to occur. c. All measures taken or to be taken to reduce, eliminate, and prevent recurrence of such an upset, slug, accidental discharge,or other conditions of noncompliance_ d. All accidental discharge reports shall be submitted to the City of Lodi, Public Works Director, 221 W.
