Order No. R5-2009-0097 - 13 - <br /> Conditional Waiver of Waste Discharge Requirements for <br /> Small Food Processors and Small Wineries <br /> Within the Central Valley Region <br /> shall submit a new Report of Waste Discharge to apply for coverage under the <br /> Waiver within 90 days of adoption of this Order. <br /> 3. Operators of small food processing or winery facilities that existed prior to adoption <br /> of this order but did not apply for coverage under the previous waiver and wish to <br /> be granted coverage under this Order shall submit a Report of Waste Discharge <br /> within 90 days of adoption of this Order. <br /> 4. New small food processors or wineries that have not begun operation as of the <br /> date of adoption of this Order shall submit a Report of Waste Discharge at least <br /> 120 days before the anticipated date of discharge. <br /> 5. To apply for coverage under this Waiver, the Discharger shall submit a Report of <br /> Waste Discharge consisting of the following: <br /> a. A completed Form 200 (Application/Report of Waste Discharge). This <br /> document and instructions for completing it may be downloaded from the <br /> Internet at <br /> http://www.waterboards.ca.gov/sbforms/form200.pdf . <br /> b. A completed Form 200-R5-FP (Report of Waste Discharge Technical <br /> Information). This document can be downloaded from the Internet at <br /> http://www.waterboards.ca.gov/centraIvaIley/board_decisions/adopted_orders/ <br /> index.shtml#Waivers <br /> c. A one-time application fee for a threat and complexity of "3C" as described in <br /> Title 23, CCR, and Section 2200. The fee shall be submitted in the form of a <br /> check made payable to State Water Resources Control Board. The current fee <br /> schedule can be downloaded from the Internet at <br /> http://www.waterboards.ca.gov/resources/fees/docs/ <br /> waste_discharge_requirement_fees.pdf <br /> Applicants are encouraged to contact Central Valley Water Board staff for <br /> assistance in determining the appropriate base fee and surcharge(s). <br /> d. If required by Central Valley Water Board staff, a chemical analysis of key <br /> waste constituents including at a minimum five-day biochemical oxygen <br /> demand, total nitrogen, pH, and fixed (inorganic) dissolved solids. <br />
