Monitor Circle Stockton,CA 95219 RE: Addendum Letter Report for Phase I Environmental Site Assessment 2128— I Oh Street Stockton,CA Dear Mr. Weisz: At your request, PIERS Environmental Services, Inc. is pleased to provide you with this addendum letter report for the Phase I Environmental Site Assessment (ESA) for the property located at 2128 — 10t' Street, Stockton, California (hereinafter referred to as the "Property"). According to a letter from the San Joaquin County Environmental Health Department (SJCEHD), dated May 3, 2004, and signed by Raymond Borges for Mike Huggins, the SJCEHD reviewed PIERS' Phase I ESA and determined that the report lacked three items required for a "Surface and Subsurface Contamination Report". This report is apparently required as part of a land use application and for parcel map approval. The following items were missing: 1) description of proposed development, 2) description of all past on-site potential and/or known above and below ground sources of contamination identified at the project site, 3) who is the true owner of the property, and who conducted the site reconnaissance, 4) the SJCEHD requires an original stamped and signed report, not a photocopy, and 5) appendices should include copies of any septic permits for the subject property. The following items are in answer to the SJCEHD comments: 1) The proposed development is for residential usage, 2) The Phase I ESA determined that there were no potential and/or known above or below ground sources of contamination. Specifically, the land was used for a residence and then was vacant. Refer to 4) concerning septic systems on-site. 3) The present owner of the property is Mr. Bill Weisz, who purchased the property on March 11, 2004; however, the property profile, which had not yet been updated, listed Mr. Robert Earl Lee(the previous owner) as the owner of record. Also, the Site Reconnaissance was conducted by Mr. Joel Greger of PIERS, for the client, Mr. Bill Weisz. 4) An original signed and stamped report is attached with this addendum letter report. 5) PIERS reviewed files for the property at the SJCEHD, and there were no septic tank permits in the file.
