Monitor Circle <br /> Stockton,CA 95219 <br /> RE: Addendum Letter Report for Phase I Environmental Site Assessment <br /> 2128— I Oh Street <br /> Stockton,CA <br /> Dear Mr. Weisz: <br /> At your request, PIERS Environmental Services, Inc. is pleased to provide you with this <br /> addendum letter report for the Phase I Environmental Site Assessment (ESA) for the <br /> property located at 2128 — 10t' Street, Stockton, California (hereinafter referred to as the <br /> "Property"). <br /> According to a letter from the San Joaquin County Environmental Health Department <br /> (SJCEHD), dated May 3, 2004, and signed by Raymond Borges for Mike Huggins, the <br /> SJCEHD reviewed PIERS' Phase I ESA and determined that the report lacked three items <br /> required for a "Surface and Subsurface Contamination Report". This report is apparently <br /> required as part of a land use application and for parcel map approval. <br /> The following items were missing: 1) description of proposed development, 2) description <br /> of all past on-site potential and/or known above and below ground sources of contamination <br /> identified at the project site, 3) who is the true owner of the property, and who conducted the <br /> site reconnaissance, 4) the SJCEHD requires an original stamped and signed report, not a <br /> photocopy, and 5) appendices should include copies of any septic permits for the subject <br /> property. <br /> The following items are in answer to the SJCEHD comments: <br /> 1) The proposed development is for residential usage, <br /> 2) The Phase I ESA determined that there were no potential and/or known above or <br /> below ground sources of contamination. Specifically, the land was used for a <br /> residence and then was vacant. Refer to 4) concerning septic systems on-site. <br /> 3) The present owner of the property is Mr. Bill Weisz, who purchased the property <br /> on March 11, 2004; however, the property profile, which had not yet been <br /> updated, listed Mr. Robert Earl Lee(the previous owner) as the owner of record. <br /> Also, the Site Reconnaissance was conducted by Mr. Joel Greger of PIERS, for <br /> the client, Mr. Bill Weisz. <br /> 4) An original signed and stamped report is attached with this addendum letter <br /> report. <br /> 5) PIERS reviewed files for the property at the SJCEHD, and there were no septic <br /> tank permits in the file. <br /> Environmental"Hot-Line"(800)559-1248 <br />
