*"W *./ <br /> NOA JOB NUMBER: E04115A <br /> PROPERLY LOCATION: 2110 East 13`x' Street,Stockton,CA <br /> Page 2 �NN <br /> The Surface/Subsurface Contamination Report Questionnaire <br /> The Surface/Subsurface Contamination Report(SSSCR) satisfies San Joaquin County requirements for parcel <br /> division and lot line adjustments and is intended to identify any sources of contamination on the property from <br /> on-or off-site causes. <br /> SanJoaquin County requires that the current(and former,in some cases)owners of the property be interviewed <br /> regarding any environmental knowledge of the property they may possess. The attached two-part questionnaire <br /> is designed to supplement the interview requirement of the SSSCR. <br /> Part 1 includes questions which relate specifically to the user's responsibilities,and to additional information <br /> sources which will be useful to us in completing our investigation most efficiently. Please complete this portion <br /> of the questionnaire at your earliest convenience and return it to us promptly. <br /> Part 2 includes questions to be asked of the current property owner and/or occupants. Please forward a copy <br /> of this portion of the questionnaire to the current owner and request that they complete and return it as soon <br /> as possible. (If you currently own the property and purchased it over six months ago,please complete Part 2 as <br /> well.
