�lzE� September 15, 2005 FOR: San Joaquin County Environmental Health Department 304 East Weber Ave.,Third Floor Stockton, CA 95202 PROJECT: 23480 Tinnin Road, Manteca, CA 95337 Service Request SR0043193 SUBJECT: Surface and Subsurface Contamination Report Addendum Reference is made to the Surface and Subsurface Contamination Report conducted for the project at 23480 Tinnin Road. Apparently,Mr. Raymond Borges of EHD has requested that one of the owners of the subject property,Mr. Tony Dutra, sign the Environmental Questionnaire included in my Report. The following is my response to this request: • First,EHD has no policy that requires an"Environmental Questionnaire"be completed for a Surface and Subsurface Contamination Report. Consequently,there is no policy as to who should sign this document. Based on their professional judgement, it should be the Environmental Professional completing the Report who decides the signer of a Questionnaire,not EHD. • It just so happens that I was standing right next to Mr. Donavan Chavez and Mr. Tony Dutra when Mr. Chavez completed the Questionnaire. Mr. Dutra suffered a severe injury to his right hand some time ago. Writing for him is difficult; consequently,Mr. Chavez completed the Questionnaire. If there were any discrepancies,I am sure Mr. Dutra would have brought them to my attention, and to Mr. Chavez's attention immediately. If I described every minute detail that has no bearing on the conclusion of a Contamination Report,my Reports would be much longer, thus requiring additional review time for understaffed and overworked EHD Sanitarians in Unit 11. • This Contamination report was submitted to EHD on July 22,2005. It has taken almost two months to review a very simplistic Contamination Report. I have spent additional,unnecessary time in preparing this Addendum, further delaying approval of the subject Report. • As stated in the second paragraph,Page 4 of the Report,"the Enviromnental Questionnaire documents environmental information directly from the people most knowledgeable about site, namely Mr. Chavez and Mr. Dutra. These two gentlemen were also interviewed for completion of this Report." This statement means that both these gentlemen completed the Questionnaire and both were interviewed. • It was also documented in the Report that Mr.Dutra and his family have owned the property for a total of 23 years.
