Other requirements may also apply. These requirements cannot be <br /> modified. <br /> A. Submit a map showing sewage information for the existing office to the Environmental Health <br /> Department showing sewage and well information that clearly delineates the proposed <br /> locations as required by ordinance requirements (San Joaquin County Development Title, <br /> Section 9-1110.4). <br /> B. If floor drains are required, they must be plumbed through a sand/oil separator and discharged <br /> to an above ground system approved by the Environmental Health Department. A registered <br /> engineer must design this system. The material collected in the above ground holding system <br /> must be analyzed for hazardous constituents prior to disposal. Specific test requirements may <br /> be obtained from the Environmental Health Department. A $260 plan check fee is required at <br /> the time•of submittal. <br /> C. The Subdivider shall send a letter approved by the Environmental Health Department to State <br /> Department of Health Services, Toxic Substance Control Division, Property Evaluation Unit. <br /> The letter is to request a determination under the provisions of Article 11, California State <br /> Health and Safety Code. The response from the Department of Health Services or the letter <br /> from the subdivider requesting that a determination be made, is to be recorded in the Title <br /> Report and the subdivision's Conditions, Covenants, and Restrictions. <br /> D. If hazardous material(s) is stored on site, applicant must register all regulated hazardous <br /> material stored onsite at, cers.calepa.ca.gov, for all regulated Certified Unified Program Agency <br /> (COPA) programs. <br /> E. If any quantity of hazardous waste is generated, a hazardous waste generator permit must be <br /> obtained from the EHD. The applicant must also obtain an EPA Identification Number, and <br /> comply with all applicable regulations. <br />
