Webber Ave,Third Floor <br /> Stockton, CA. 95202-2708 <br /> RE: 1" Quarter 2004 Septic Tank Monitoring <br /> Commercial Septic Permit#SR0033926 <br /> Dear Mr. Huggins, <br /> In a letter dated June 6, 2003, the San Joaquin County Environmental Health <br /> Department (EHD) requested that a monitoring program be implemented for the newly <br /> constructed and expanded septic system regulated under commercial septic permit# <br /> SR0033926. The purpose of this report is to fulfill that requirement, and as requested in <br /> the June 6, 2003 letter,we will continue quarterly monitoring and reporting on the status <br /> of the septic system. <br /> Background <br /> 'I lie system was Installed by Applegate Construction on or about May 30, 2003 and <br /> designed by Neil 0. Anderson and ,yssociatcs, Inc. When Neil CIAnderson conducted <br /> a joint final inspection with EHD, several problems were found including leach lines and <br /> distribution boxes out of level, and cracked and open bottomed boxes. For a summary <br /> of the construction,problems, and corrective actions, please see the attached letter of <br /> final inspection, written by David Welch and Neil O. Anderson. The installation and <br /> final inspection was completed on June 26, 2003. <br /> Monitoring Points <br /> Monitoring was to begin approximately six months after beginning operation of the <br /> system. Samples were drawn from the distribution boxes prior to the leach fields and <br /> transmitted to the laboratory under chain of custody control. <br /> The program requires three total monitoring points. The monitoring points are located <br /> in each of the three distribution boxes, just prior to discharge to the leach fields.
