The attached two- <br /> part questionnaire is designed to supplement the interview requirement of the SSCR. <br /> LPart 1 includes questions which relate specifically to the user's responsibilities,and to additional information <br /> sources which will be useful to us in completing our investigation most efficiently. Please complete this <br /> Lportion of the questionnaire at your earliest convenience and return it to us promptly. <br /> Part 2 includes questions to be asked of the current property owner and/or occupants. Please forward a <br /> copy of this portion of the questionnaire to the current owner and request that they complete and return it as <br /> soon as possible.
