SC 004 10/02/2015 Septic tank was pumped out as required by NTA. PE 4200 ESCOTTO 30 min SC 004 01/31/2018 Ardelle 5044. Issued 2nd NAI to obtain permit to destroy tank and connect to the PE 4200 WONG 60 min sewer. Donald has dug a trench to the edge of the street attemtping to connect to SC 004 sewer by himself. Told Donald to stop immediately. Asked if he has spoke to City Muni yet; he said he got confused after being sent to multiple places. Called COS Muni Jeff Telmon at 937-8341 and left him a vm of what Donald is trying to do. 01/31/2018 Ardelle 5044. Office time. see s/c 004 entry PE 4200 WONG 30 min SC 007 02/01/2018 Ardelle 5044. Mr. Belvins has already taken out permit to destroy the tank on PE 4200 WONG 60 min 11/9/2015 under SR0073591. Returned Jeff Telmon of COS Muni call; fwd him SC 004 the photos. Sewer is actually provided by ESSSP. 03/02/2018 Ardell 5044. Verified tank destruction permit SR0078739 taken out on 2/13/18. PE 4200 WONG 12 min Will wait till owner call for inspection after he connects to the city. SC 007 03/05/2018 Ardell 5044. DA Referral; described complaint to Michelle H., and made copy of PE 4200 WONG 60 min entire complaint plus past paperwork/permits. Left voice mail with Donald inquiring SC 007 status with sewer connection. Donald called back and said he plans to go to the City this next 2 weeks to get an ok to connect. Flag 3/19/18 03/06/2018 Ardell 5044. Found copy of SR0078739. Made copy for Michelle H. for DA PE 4200 WONG 12 min Referaal. SC 007 03/12/2018 Ardell 5044. Withdrew DA Referral to Michelle H. for now. Told Donald I'll not DA PE 4200 WONG 30 min Refer yet, but he has to give me a time when he will connect to city and complete SC 007 SR9978739 to destroy the septic tank. He promised to call me by end of this week. 03/16/2018 Ardell 5044. Don called and said he'll have to get back to me next week as he PE 4200 WONG 18 min was sent to different departments in his attempt to get ok to connect to city, and he SC 007 has difficulty traveling. I'll give him till next week. 03/29/2018 Ardell 5044. Returned Don's call; he's still unable to get the permission to PE 4200 WONG 24 min connect: John Wotila (sp?) of Dept of Multiple Utilities 937-8436 told Don he has SC 007 to show them the deed; Don can't find it and has to go to a Trust(?) and get a notorized letter Downtown and he can't do that at this time because it will cost about$700 and he gets only about#700 from retirement/disability. Told him to keep working on it and give me an ETA, and in the mean time cover up the hole over th a tank if he hasn't already to prevent anything surfacing or brothering the neighborhood with odor. In either case I will call him back in 2 weeks on status. 04/18/2018 Ardell 5044. Don said he still hasn't got the ok to connect with the City Muni: he PE 4200 WONG 30 min still has to notarize Condition sewer connection agreement with the City, and pay SC 007 the fee. Spoke to John Walktilla (937-8436) and confirmed Don's submitted the application, so just the fee and notarized agreement is missing.
