S A N 3J O A Q U I N Environmental Health Department <br /> -COUNTY- - <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> STOCKTON LOGISTICS LLC T4199 E GIBRALTAR CT, STOCKTON September 12, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1)Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> 55 gallons of Waste Oil, approximately 130 Forklift Lead-Acid Batteries with unknown amount of Sulfuric Acid, <br /> unknown amount of Liquid Detergent, unknown amount of Bleach, unknown amount of solid Detergent, unknown <br /> amount of Spray Aerosols, unknown amount of flammables( Charcoal Lighter Fluids, and Matches), unknown <br /> amount of Insecticides,three diesel generators with 925 gallons of Diesel, and 165 gallons(three barrels of Oil, <br /> Lubricant,and surfactant)were found on site that have not been reported to CERS. <br /> Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br /> Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, add the <br /> materials to the hazardous material inventory, and submit to the EHD for approval. <br /> This is a repeat violation, Class Il. <br /> 7 HSC 25505(a)(3)Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures have incorrect information. The local Unified Program Agency(UPA) Phone <br /> number and the San Joaquin County General Hospital are listed as(209)468-3451; this number is incorrect, and the <br /> correct number to the UPA/San Joaquin County Environmental Health Dept. is(209)468-3420 and to San Joaquin <br /> County General Hospital is(209)468-6000. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to, the following: <br /> immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property,or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class ll. <br /> 11 HSC 25505(a)(4)Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> There were no records of emergency response procedures training, evacuation drills, and ammonia awareness <br /> training at the time of the inspection. <br /> The facility shall have training programs for all employees and annual training, including refresher courses, shall <br /> include safety procedures for the event of a release or threatened release of a hazardous material, including, but not <br /> limited to, familiarity with the facility's emergency response plans and procedures. . This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Onsite to conduct a Hazardous Materials Business Plan (HMBP) inspection.
