Documentation showing fees have been paid has not been received. <br /> • Violation #6,the site map does not show or address the following: <br /> 1. Loading areas. <br /> 2. Internal roads. <br /> 3. Adjacent streets. <br /> 4. Storm and sewer drains. <br /> 5. Emergency response equipment (fire extinguishers and spill control). <br /> 6. Emergency shutoffs. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br /> and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". <br /> • Violation #12, remains open until violation 6 has been addressed. <br /> As of September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
