The site map was missing a key or legend to define the <br /> acronyms PL, STMD, and Fire X. The site map did not list the location of the utilities shut offs for power and water. <br /> Natural gas is not located at this site. Please add the missing elements to the site map. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as"NA".This violation was corrected on site. <br /> This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not complete. The local Unified Program Agency(UPA)phone number <br /> was not provided on line C1 And Line E3 was missing the location of the evacuation staging area. Please add the <br /> missing elements to the consolidated emergency response and contingency plan. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> This violation was corrected on site. <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually. The business plan was submitted late on 9/16/2019. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year.This violation was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> All violations have been corrected on site. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
