Box 809046 <br /> Dallas, TX 75380-9046 <br /> Dear Mr. Poole: <br /> CLOSURE PLAN APPROVAL, OVERHEAD/LODI DOOR COMPANY, <br /> EPA I.D.#CAD009174673 <br /> The Department of Health Services (DHS) has reviewed the Closure <br /> Plan, dated January 1990 , for the closure of the hazardous waste <br /> surface impoundment at the Lodi facility of Overhead Door in San <br /> Joaquin County. <br /> Pursuant to Title 22 , California Code of Regulations (CCR) , your <br /> Closure Plan is determined to be acceptable and is hereby <br /> approved. The Central Valley Regional Water Quality Control <br /> Board (CVRWQCB) has reviewed the Closure Plan and the <br /> Hydrogeologic Assessment Report for compliance with Health and <br /> Safety Code, Article 9 .5. They will issue a final report on the <br /> surface impoundment closure and monitoring well closure. Please <br /> note that backfilling of the pond can not occur until the health <br /> risk assessment, performed by a DHS toxicologist, is completed. <br /> Upon completion of closure, you and a registered professional <br /> engineer must certify that closure was conducted in accordance <br /> with the approved plan.
