MONITORING AND REPORTING PROGRAM NO. 90-803 -3- <br /> DALLAS CORPORATION, DBA OVERHEAD/ <br /> LODI DOOR COMPANY, SAN JOAQUIN COUNTY <br /> REPORTING <br /> 1 . In reporting the monitoring data, the Discharger shall arrange the data in <br /> tabular form so that the date, the constituents, and the concentrations <br /> are readily discernible. The data shall be summarized in such a manner to <br /> illustrate clearly the compliance with this monitoring order. <br /> 2. Quarterly monitoring reports shall be submitted to the Regional Board by <br /> the 15th day of the following month. <br /> 3. The results of any monitoring done more frequently than required at the <br /> locations specified in the Monitoring and Reporting Program shall be <br /> reported to the Board. <br /> 4. Reporting of monitoring shall include copies of field data, to be signed <br /> by the person performing sampling, chain of custody forms, and copies of <br /> lab reports. <br /> 5. The water level information required in item 1 , page 1, shall be presented <br /> in a table along with the reference point elevation, the ground surface <br /> elevation for each well , and the date and time the water level <br /> measurements were taken. An accurate site map should be included showing <br /> the water level elevation at each well location, and the magnitude and <br /> direction of the ground water gradient. The gradient should be calculated <br /> twice, using two different combinations of the five wells, MW-1 , MW-2, MW- <br /> 3, MW-4 and MW-5. <br /> 6. An annual cation-anion balance shall be calculated from the <br /> milliequivalent concentrations determined in ite 3, page 2. <br /> Discrepancies in the balance of greater than 5% shall a addressed, but <br /> this shall not delay the initial submittal of the monitoring results. <br /> 7. Annual reports shall be submitted summarizing the year-long monitoring <br /> results, discussing trends and changes in water quality, and discussing <br /> variations in the magnitude and direction of ground water flow over the <br /> course of the year. This report shall include recommendations for further <br /> monitoring. The annual report shall be prepared and signed by a State of <br /> California registered civil engineer, registered geologist, or certified <br /> engineering geologist. <br /> WILLIAM H.
