' CLOSURE REPORT <br /> Monitoring Well Abandonment <br /> ' Overhead Door Facility <br /> Lodi, California <br /> ' 1.0 INTRODUCTION <br /> BSK & Associates, Inc. (BSK) has prepared this Closure Report for the abandonment of <br /> ' six on-site groundwater monitoring wells. BSK was the prime contractor (License Number <br /> C57-490942) and Halliburton Services was a subcontractor. <br /> I The location of the site within the Lodi area is shown on Figure 1, Vicinity Map. The job <br /> site address is 1220 East Victor Road in Lodi, California. The locations of the six <br /> groundwater monitoring wells abandonment are presented on Figure 2, Site Plan. <br /> 1.1 Work Plan and Site Safes Plan - Prior to permitting the work, a Work Plan <br /> and Site Safety Plan dated May 19, 1992 was prepared by BSK and submitted <br /> to Remediation Services, Inc. (RS). The Work Plan and Site Safety Plan <br /> included abandonment methodology, potential chemical and physical hazards, <br /> the grout mix design, and a project schedule. Two copies of the Work Plan <br /> and Site Safety Plan were submitted to Overhead Door Corporation by RS on <br /> May 19, 1992. A copy of the Work Plan and Site Safety Plan was forwarded <br /> to the Regional Water Quality Control Board (RWQCB) by Overhead Door <br /> Corporation. <br /> 1.2 Permits - Prior to commencement of work, a destruction permit application, <br /> a remittance check for $60.00, and a copy of the Work Plan and Site Safety <br /> Plan prepared by BSK was submitted to Ms. Diane M. Hinson of the San <br /> ' Joaquin County Public Health Services, Environmental Health Division <br /> (SJPHS) on May 19, 1992. A copy of the cover letter and signed acceptance <br /> and completion permit for the destruction of six groundwater monitoring wells <br /> ' is attached under Appendix A. On May 28, 1992 BSK was informed by Mr. <br /> Michael J. Infurna, Jr., of SJPHS that our permit application was approved. <br /> 1.3 Site Preparation - Arrangements were made with Mr. Craig Edwards of <br /> Overhead Door (Sacramento) on May 28,1992 to gain access to the Lodi <br /> Facility for the following day. Also, BSK informed Mr. Michael J. Infurna, Jr. <br /> ' that work would start on June 1, 1992. <br /> On May 29, 1992 two representatives of BSK cemented a 2%,-foot long, 8-inch <br /> ' diameter PVC schedule 40 riser inside of each monitoring well vault box with <br /> a quick-set grout mix. The riser installations are illustrated on Photographs <br /> #2 and #3, Figure 3; Photographs #10, #14, and #15, Figure 4; and <br /> ' Photographs #18, #19, and #20, Figure 5. <br /> 1.4 Mobilization/Demobilization - BSK and Halliburton mobilized and were on- <br /> site at 08:00 on June 1, 1992.
