Please make a <br /> submittal for 2019. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/,enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 10 HSC 25508.1(a)-(f)Failed to submit revisions to business plan within 30 days of substantial change in operations. <br /> During the inspection, it was found that carbon dioxide has been handled on site and the updated information has <br /> not been submitted. Within 30 days of one the following events, business shall electronically update the information <br /> submitted to the California Environmental Reporting System(CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership,or business name. <br /> Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/,enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class Il. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were unavailable for review at time of inspection.The business plan shall include <br /> provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures,the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
