A N J O A Q U I Environmental Health Department <br /> COUNTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: FDat,-.TAQUERIA LA ESTRELLA 1110 N MAIN ST, MANTECA ber 11, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 10 HSC 25508.1(a)-(f) Failed to submit revisions to business plan within 30 days of substantial change in operations. <br /> During the inspection, it was found that the carbon dioxide has been handled and the updated information has not <br /> been submitted. <br /> Within 30 days of one the following events, business shall electronically update the information submitted to the <br /> California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available for review at time of inspection. <br /> The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual <br /> refresher training. This training shall be documented electronically or by hard copy and shall be made available for <br /> a minimum of three years. Immediately provide employee training for appropriate personnel and submit a copy of <br /> the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c)Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. <br /> The emergency response plans and procedures,the inventory of information required by this article, and the site <br /> map required by this section shall be readily available to personnel of the business or the unified program facility <br /> with responsibilities for emergency response or training pursuant to this section.
