SECOR www secor coin <br /> ® INTERNATIONAL 3017 Kilgore Road Suite 100 <br /> INCORPORATED Rancho Cordova CA 95670 <br /> 916.861-0400 TEL <br /> 916-061-0430 FAx <br /> March 11, 2004 I F,C�[E W E D <br /> Mr Harlin Knoll MAR 15 2004 <br /> San Joaquin County ENVlkuNMENT HF-A TH <br /> Environmental Health Department PERMET/SE;-)1.121, J <br /> 304 East Weber Avenue, Third Floor <br /> Stockton, CA 95202 <br /> RE Well Destruction Report <br /> 7-Eleven Store#17647 <br /> 1048 West Yosemite Avenue <br /> Manteca, CA <br /> SECOR Project No 77EL 17647 03 0512 <br /> Dear Mr Knoll <br /> This report is prepared by SECOR International Incorporated (SECOR) on behalf of 7-Eleven, Inc to <br /> document the destruction of three groundwater monitoring wells located at 1048 West Yosemite <br /> Avenue in Manteca, California (Figures 1 and 2) The report has been prepared in response to a <br /> California Regional Water Quality Control Board (RWQCB) letter dated October 30, 2003 requesting <br /> that the site-related groundwater monitoring wells be destroyed as part of the final closure <br /> requirement (Attachment A) <br /> Well Destruction <br /> Permitting/Utility Clearance - SECOR obtained well destruction permits from the San Joaquin <br /> County Environmental Health Services Department (SJCEHD) (Attachment A) Prior to conducting <br /> subsurface work at the site, Underground Service Alert (USA) was contacted to delineate subsurface <br /> piping near the site with surface markings In addition, a private utility locator service was contracted <br /> to clear each well location <br /> Health and Safety - SECOR modified our existing site-specific Health and Safety Plan for the scope <br /> of work as required by the Occupational Health and Safety Administration (OSHA) Standard <br /> "Hazardous Waste Operations and Emergency Response" guidelines (29 CFR 1910 120) r,,The <br /> document was reviewed and signed by SECOR personnel and subcontractors performing work at <br /> the site <br /> Well Destruction — On January 23, 2004, SECOR supervised the destruction of three site <br /> groundwater monitoring wells by V & W Drilling, Inc Monitoring wells MW-1, MW-2 and MW-3 were <br /> destroyed by pressure grouting each well from total depth to five feet below ground surface (bgs) <br /> Wells MWA and MW-3 were overdrilled three feet bgs and the well box and concrete apron were <br /> removed With the approval of the SJCEHD representative, monitoring well MW-2 was not <br /> overdrilled to three feet bgs due the presence of overhead utility lines MW-2 was pressure-grouted <br /> from total depth to top of casing and the well box and concrete apron removed <br /> After each well box and concrete apron was removed, the borings were filled with neat cement grout <br /> and capped with either concrete or asphalt to match existing grade The well boxes, concrete <br /> aprons and soil debris were temporarily stored in a 55-gallon drum onsite Copies of the field notes <br /> summarizing well destruction activities are provided in Attachment B <br /> 17647 Well Destruction Report <br />
