y SH J O A Q U I N Environmental Health Department <br /> CSU � 1 Y <br /> Complaint Record Daily Time and Activity Summary from 9/3/2019 to <br /> 10/14/2019 for C00050417 located at <br /> DELICATO VINEYARDS <br /> 12001 S HWY 99, MANTECA <br /> Activity Date Recorded by <br /> 09/04/2019 Received a voicemial at 9:55am from Lisa Luis (209)824-3488 stating that they had PE 2546 SAEED <br /> 180 min an an ammonia release this morning and she is notifying us. At 11 am I called lisa SC 544 <br /> and told me this morning at 1:30am while a cellar technician was hooking up hoses <br /> to wine tank 1508, he sniffed an ammonia. The cellar technician reported the <br /> incident. Around 1:40am the Maintenance personnels isolated the ammonia to the <br /> tank, and started the investigation. There was no evacuation or exposure due to the <br /> release. The insulation on the tank was removed to determine the point of the <br /> release on the tank. It was discovered there is a dimple on the tank on spot welded <br /> area. It is unknown amount of an ammonia vapor released. The wine tank is located <br /> outdoor. The facility will fix the dimple on the tank, pressure test the tank, and <br /> re-insulate the tank before running it again. <br /> I told Lisa to follow their emergency response/Action plan and call the the EHD <br /> main number(209)468-3420. 1 asked lisa why the incident was not reported right <br /> away, and told me she is not sure. <br /> I discussed the incident with Muni, and confirmed that I have to response. I called <br /> Lisa and confirmed she will be there around 12pm. <br /> I left the office at 11:35am and arrived onsite at 11:50am. <br /> I met with Lisa and walked to the tank. The tank has about 2 x5 feet of insulation <br /> removed to determine the point of leak. No sign of corrosion on the tank. The <br /> release was determined to be from a welded spot on the tank. The ammonia supply <br /> to the tank is isolated, and the wine inside the tank was evacuated until the the tank <br /> is repaired. I took photos of the tank and the insulation removed. <br /> discussed with Lisa generating Cal OES report. I left the site at 12:23pm. <br /> I filled out the complaint info, generated the notice to abate and emailed to Lisa <br /> Luis. <br /> I generated the emergency response records, prop 65, photo log and printed copies. <br /> 10/14/2019 Received the incident investigation report from Lisa Luis on 10/14/2019 for incident PE 2546 SAEED <br /> 60 min occurred on 09/04/2019. 1 reviewed and found it incomplete. I emailed Lisa today SC 544 <br /> stating, "Thanks Lisa. Recommendation#2 states there is a work order attached to <br /> the report. The WO is no attached! Can you please email me a copy?Also, you can <br /> update the Cal OES spill report with the quantity calculated. Let them Know this is <br /> an update" <br /> I received the work order, and the initial incident report from Lisa. I reviewed and <br /> printed a copy to attach to the report. Also, the Cal OES spill report was updated, <br /> and attached a copy to the report. <br /> I printed the narratives, and attached to the report. I abated the complaint report and <br /> sent it to the counter staff to be filed. <br /> Page 2 of 2 <br />
