This <br />is a Class II repeat violation. <br />110 <br />Failed to submit secondary containment test report within 30 days. Secondary containment testing was scheduled to be <br />performed on 11/21/2013 and a test report has not been submitted to the EHD. A copy of the test report must be <br />submitted within 30 days of the test. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov and upload a copy of the test report as a miscellaneous State -Required Document in the <br />Underground Storage Tank section or schedule this test and provide 48 hours notification to the EHD. A copy of the <br />report was provided to owner by Affordatest during this inspection, and EHD accepted a photo copy of it. This violation <br />has been corrected on site. This is a Class II repeat violation. <br />124 <br />Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on <br />9/23/2013 and an inspection report was issued identifying information to be submitted to bring this site into compliance. <br />This information was required to be submitted by 10/26/2013. This information for CERS submittal requirement (#999, <br />HSC 25404(e)4) has not been received resulting in a non-compliant status for this facility. An operator that receives an <br />inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br />actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br />corrective actions taken or proposed is submitted within 30 days of receiving an inspection report.
