EHD still has not received <br />the following information, resulting in a non-compliant status for this facility: <br />- Alarm logs for last year (see violation #201 on the inspection report) <br />- Current designated operator (D.O.) is conducting monthly inspection (#204) <br />- Alarms that were not recorded on last twelve months of D.O. reports (#204) <br />- A copy of the missing July and August D.O. inspection reports (#205) <br />- A copy of the current training records of employees other than Alam Zafar (#206) <br />- Repair the monitoring panel by a license, trained, and certified contractor, and its functionality verified <br />under EHD inspection. (#301, 303) <br />- Complete California Environmental Reporting System (CERS) electronic submittal requirement (#999) <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is <br />billed at the current hourly rate ($125). Failure to submit the completed information immediately <br />may result in a re -inspection, additional violations, and/or further legal action. <br />Immediately submit the above -referenced information to EHD by January 10, 2014.
