The following violation #105 is a <br />Minor -Notice to comply, and the #311 is a Class II Repeat violation. <br />105. UST tank forms on file with the EHD are not current: All three tank forms listed the riser, as well as <br />vent and vapor -return piping to be double -walled steel construction, which EHD inspector was unable to <br />verify for the riser during this inspection. Any change of information on these forms must be submitted <br />to the EHD within 30 days of the changes. Immediately complete and submit a copy of the facility and <br />tank forms to the EHD by 10/24/2012. Complete a separate tank form for each tank. <br />311. (Repeat) The 87 -octane VMI line leak detector failed to detect a leak when tested, even after the <br />service technician adjusted the leak detector and retested it. All line leak detectors shall be capable of <br />detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and certified <br />contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and obtain a permit <br />within one business day from the EHD. If the failed leak detector can't be replaced immediately, there is <br />a possibility that the 87 -octane UST system may be red tagged to prevent fuel inputs. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EH ' URRENT HOURLY RATE. <br />
