The permit has been <br />approved with the followng conditions <br />1_ Contact the local fire department and the Sen Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in Information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3- ContactJeffWong at (209) 4684335 to arrange the following inspection appointments <br />(a 48-hour noticeis reamed for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Leak detector teat Leak detector must be tested to confirm that the device can <br />detect a leak of 3 gallons per hour at 10 psi. <br />h_ Overfill prevention verification: Verify that external audio and visual alarm activates <br />when lank is 90% full. <br />c. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test (or oquwalent). Proper drainage must also be verified. <br />Please be aware that the facility cannot operate the UST system tank prior to the <br />EHD inspection and approval. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physlcaffy presets at the work site who prssassas all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by Bre <br />Contractors State Ltcanse Baard (may be issued to the instvllation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />b- A valid, current California UST Service Technician certificate issued by the <br />international Code Council (ICC), and <br />
