Nicole Carter from The PE 2546 BAKER <br /> 18 min Wine Group (17000 E HWY 120) reported a small ammonia leak. SC 907 <br /> 8:54 am called Nicole Carter at 209-608-1915. She estimated that the leak was <br /> approximately 2 pounds. The leak is stopped and was caused by the bull's eye <br /> sight glass high pressure receiver. No injuries or evacuations as a result of the <br /> leak. Unsure of when it will be fixed. <br /> 8:57 am called Muni Naidu (Program Coordinator)to discuss complaint. He said <br /> to tell them to call when the leak is fixed or if anything changes. <br /> 9:01 am- Called Nicole Carter back. She said that she was just about to call me <br /> because the part was fixed already. I asked if it was a gas or liquid leak. She said <br /> gas and that all the notifications have been made. <br /> 8:48 am -9:06 am. <br /> 10/14/2019 Generated complaint. Completed prop 65. SC 544 <br /> Gave complaint to Haza Saeed (Lead Sr. REHS). <br /> 10/14/2019 Lydia Baker with EHD discussed with me that she received this complaint on PE 2546 SAEED <br /> 60 min 10/12/2019. 1 called Nicole Carter today at 9:45am to obtain more information. The SC 544 <br /> leak came from one of Bulls Eyes on the Area B Ammonia Receiver. The Bulls <br /> Eye has no corrosion or damage according to Nicole. The reason for the leak <br /> could be because it is loose Nicole stated. The Bulls Eye was tighten to fix the leak <br /> on the same day. The leak estimated to be 2 pounds and calculation and incident <br /> investigation in progress. No exposure or evacuation resulted from the leak. I told <br /> Nicole that I will discuss the incident with my manager, and let them know if I will <br /> be on site for inspection. She told me that they have videos and photos that she <br /> can share with me. <br /> I discussed with Muni N. the incident. I asked Muni if I should respond. Muni asked <br /> me what caused the release, andl explained. Muni told me we could audit the <br /> facility. I told him the releases at this facility are not due to Mechanical Integrity <br /> issues, such as corrosion. I told Muni the facility told me they have videos and <br /> photos they can send me and I can email them a notice to abate to send their <br /> incident investigation report. I told Muni that the Bulls eyes was loose , and Muni <br /> asked why, and told him this issue is still under investigation. Muni told me to state <br /> the notice to abate if the incident investigation report is unsatisfactory we could <br /> audit the facility, and I added it to the report. <br /> I generated the Notice to abate, and emailed to Kevin Jones and Nicole Carter. I <br /> asked them to email me the Photo/video they took, and submit the incident <br /> investigation report by 11/13/2019. 1 Printed a copy of the notice to abate report <br /> and attached to the complaint report. <br /> Page 1 of 2 <br />
