The release is stopped and contained. <br /> No Waterways were impacted. The Wine Group performed the clean-up. <br /> OBSERVATIONS: <br /> A Cellar employee noticed an ammonia leak while performing his duties in the area. The release occurred at the Bulls Eye <br /> glass located on the Area B Ammonia Receiver. No exposure or evacuation reported due to the release. The Bulls Eye <br /> glass was tighten and the issue was fixed. The release was contained and stopped.Approximately 2 lbs of ammonia were <br /> released. The Area B ammonia receiver is back in operation. Incident investigation is in prociress. <br /> VIOLATIONS: <br /> Discharge of any waste, including sewage <br /> (CA Health and Safety Code 5411) <br /> Discharge of waste or sewage to water or within 150 ft. of waterway <br /> (SJC Ordinance Code Sections 9-1125.3, 9-1125.4) <br /> Improper disposal of a hazardous waste <br /> (CA Health and Safety Code 25189.5) <br /> • Permitting or Maintaining a Nuisance <br /> (SJC Ordinance Code Section 8-5100) <br /> Other Violation <br /> ORRECTIVE ACTIONS/ORDER: <br /> • Investigate and abate contamination under the oversite of the appropriate lead agency. <br /> Submit copies of hazardous waste manifests/disposal records to the EHD. <br /> • Other Correction <br /> Submit the incident investigation report to SJC EHD by November 13,2019. <br /> Update the Cal OES spill report as needed. <br /> Note: SJC EHD may audit the RMP program if the incident investigation report is unsatisfactory. <br /> EHD 25-01 Rev.06/17/17 Page 1 of 2 WASTE DISCHARGE/SPILL RESPONSE NOTICE TO ABATE <br /> 1868 E.
