Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #1 Demonstration of Knowledge <br /> OBSERVATIONS:3 food handlers do not have food handler's cards. Provide in 30 days. This is a repeat violation. <br /> CALCODE DESCRIPTION:All food employees shall have adequate knowledge of and be trained in food safety as it relates to their <br /> assigned duties. (113947-113947.1)Any food handler hired after June 1, <br /> 2011 shall obtain a Food Handler Card within 30 days(113948). <br /> #23 Rodents, Insects or Animals Inside Facility <br /> OBSERVATIONS:Flies in the backroom, and dead roaches in glue trap in restroom. Remove roaches and provide more <br /> pest control; keep doors closed to prevent access to flying insects. This is a repeat violation. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> #29 Toxic Substances Properly Identified/Stored/Used <br /> OBSERVATIONS:Two bottles of boric acid in restroom. Remove from facility immediately and use only non-toxic means of <br /> pest control.This is a repeat violation. <br /> CALCODE DESCRIPTION:All poisonous substances, detergents, bleaches, and cleaning compounds shall be stored separate from food, <br /> utensils,packing material and food-contact surfaces. (114254, 114254.1, 114254.2) <br /> #45 Floors,Walls, Ceilings; Clean and Maintained <br /> OBSERVATIONS:Missing baseboard on west wall of backroom and 2-comp sink, gap/warpped/crack to baseboard <br /> under 2-comp sink, and crack on vinyl floor of backroom. Repair and seal to prevent water intrusion and access to vermin, <br /> and provide cleanable and non-absorbent surface in 30 days. <br /> Some food debris and oil on floor in backroom, and food debris on floor at counter. Clean in 1 day. <br /> CALCODE DESCRIPTION: The walls/ceilings shall have durable,smooth,nonabsorbent,light-colored,and washable surfaces. All floor <br /> surfaces, other than the customer service areas, shall be approved, smooth, durable and made of nonabsorbent material that is easily <br /> cleanable.Approved base coving shall be provided in all areas,except customer service areas and where food is stored in original <br /> unopened containers. Food facilities shall be fully enclosed. All food facilities shall be kept clean and in good repair.
