Appeals must be filed with this Department within 10 days of the action with an appeal fee <br /> of$648.00. The 10-day appeal period ends at 4:30 p.m.on October 13, 2019. If this date falls on a weekend <br /> of holiday, the appeal period will expire on the next regular business day at 4:30 p.m. <br /> EXPIRATION: This action requires you to comply with all of the original Conditions of Approval by August <br /> 14, 2025, the new expiration date.
