A representative from Aswas signed the Site Mitigation Master Record Form; a faxed copy is <br /> included with this application package. The original will be submitted to you next week. The <br /> Masterfile Record Information Form has been completed, specifying Montgomery Watson as the <br /> billing party for San Joaquin County Public Health Services--Environmental Health Division <br /> (PHS-EHD) charges associated with the proposed soil gas vapor survey project at the Site. Finally, a <br /> well permit application form has been completed. Transglobal Environmental Geochemistry (TEG), <br /> a C-57 license holder, is scheduled to collect the soil vapor samples. <br /> The work plan describing the scope of work to be conducted at the Site is also enclosed. Based on <br /> our phone conversation earlier today, a check in the amount of$333.00 made payable to San Joaquin <br /> County is enclosed. This amount covers 3 hours of PHS-EHD time for processing the application (at <br /> $78.00 per hour) and$89 for the permit. <br /> I sincerely appreciate your time and consideration.
