A representative from Aswas signed the Site Mitigation Master Record Form; a faxed copy is
included with this application package. The original will be submitted to you next week. The
Masterfile Record Information Form has been completed, specifying Montgomery Watson as the
billing party for San Joaquin County Public Health Services--Environmental Health Division
(PHS-EHD) charges associated with the proposed soil gas vapor survey project at the Site. Finally, a
well permit application form has been completed. Transglobal Environmental Geochemistry (TEG),
a C-57 license holder, is scheduled to collect the soil vapor samples.
The work plan describing the scope of work to be conducted at the Site is also enclosed. Based on
our phone conversation earlier today, a check in the amount of$333.00 made payable to San Joaquin
County is enclosed. This amount covers 3 hours of PHS-EHD time for processing the application (at
$78.00 per hour) and$89 for the permit.
I sincerely appreciate your time and consideration. If you should have any questions, call me at
(510)975-3473.
Thank you,
Chris Chan
Montgomery Watson
