4.0 SAFETY AND HEALTH RISK EVALUATION <br /> 4.1 Potential Physical Hazards <br /> Field personnel should be cognizant of potential physical hazards associated with use of heavy <br /> equipment and electrical equipment during field operations and traffic. Appropriate precautions <br /> include the following: <br /> ANSI-approved hardhats, safety glasses or goggles, and steel-toe boots will be <br /> worn. <br /> Loose clothing that may catch in moving parts will not be worn. <br /> Hearing protection will be worn if a preliminary noise survey or past experience <br /> indicates that maximum noise levels will exceed 85 decibels at any time during <br /> site operations. <br /> Additionally, field personnel should not enter any excavations exceeding 5 feet in depth unless <br /> the excavations are property shored, braced or sloped and a safety ladder is provided for ready <br /> access or egress. <br /> K/J personnel will not enter any confined space defined by OSHA as the concurrent existence of <br /> the following conditions without advanced specific preparation, planning, training, and <br /> supervision by the Regional Safety Supervisor. <br /> 1. Existing ventilation is insufficient to remove hazardous air contaminants and/or <br /> oxygen deficiency exists. <br /> 2. Ready access or egress for the removal of a suddenly disabled employee is <br /> difficult due to location and/or size of the opening. <br /> 3. An atmosphere presenting a threat of causing death, injury, acute illness or <br /> disablement exists. <br /> Adverse climate conditions, primarily heat, are important considerations in planning and <br /> conducting site operations. Maximum daytime temperature may exceed 75 degrees F at the site <br /> and heat stress is an associated concern. Preventative measures should include the following: <br /> Frequent rest periods in the shade.
