CITY OF LODI ENCROACHMENT PERMIT CONDITIONS <br /> ATTACHMENT TO ENCROACHMENT PERMIT#2012-062 <br /> NOTIFY CITY INSPECTOR AT START OF WORK 209-333-6706 <br /> WORK IN PUBLIC RIGHT OF WAY <br /> 1. Barricades required at work site. No open excavations except at time of work. <br /> Use steel plates or backfill with A.C. cap. Match finish grades. Affected portions <br /> of public sidewalk to be closed in vicinity of work in progress;proper signage <br /> required. "Sidewalk Closed Ahead"signs required at nearest crosswalks. Work <br /> site signage per Caltrans Traffic Control Manual, current issue; <br /> 2. Street structural backfill per Std Plan no. 506; no shoulder patch(T-Patch) <br /> required. Pavement excavations shall be squared. Edges of excavation patches <br /> adjacent within 5-feet of one another shall be treated as one pavement patch. <br /> Excavation s shall be contained within parking lanes where possible. Excavations <br /> located in travel lane shall require pavement replacement to centerline of street. <br /> Excavations within 12-inches of toe of gutter shall be backfilled per Std Plan 505. <br /> Match finish grades; <br /> 3. Alley excavation pavement backfill shall be a minimum of 3-inches asphalt <br /> concrete. Match finish grades; <br /> 4. Sidewalk replacement and repair per Std. Plan No. 117, 121 and 135; match finish <br /> grades; all work shall comply with City of Lodi Standard Plans and Construction <br /> Specifications, <br /> 5. Removal of paint markings on concrete sidewalk to be hydro/sandblasted, <br /> wastewater run-off to be vacuumed. <br /> 6. No foreign material to enter City storm drain system; no flushing;no <br /> sweeping into gutter. Filter screens per Std. Plan No. 313 required and <br /> maintained at nearest catch basins. Wastewater generated by concrete work and <br /> or debris to be captured and vacuumed prior to entering city storm drain system; <br /> 7. Work site and adjacent street pavement to be kept in a clean and neat manner. <br /> Accumulation of silt, sand or other material on street pavement will not be <br /> permitted and must be routinely removed. <br /> 8. No open excavations except at time of work. No foreign material to enter City <br /> storm drain system;no flushing, no sweeping into gutter. Accumulation of sand <br /> or other material,which might interfere with drainage, constitute a hazard to <br /> traffic and will not be permitted and must be removed. <br /> 9. No material to be stockpiled in street or sidewalk. Use of private property shall <br /> be only at permission of property owner. <br /> 10. Traffic control for work within the public right-of-way, including overhead <br /> operations, shall comply with Caltrans Traffic Control Manual, current issue. <br /> Public Streets to remain open at all times. Precautionary measures shall be taken <br />
