A.P.N. 093-330-10, Property Address 9156 North Shelton Road: <br /> SUBJECT: Proposal for a Surface and Subsurface Contamination Report per Project <br /> Description and Conditions of Approval from Public Health Services <br /> Dear Mr. and Mrs. Hawley: <br /> Thank you for contacting me to submit this proposal for providing a Surface and Subsurface <br /> Contamination Report (SSCR). As Mike Huggins of the Environmental Health Department has <br /> written to you about, this report is required by the Health Department. The contents of my <br /> proposal are based upon the written requirements of the Health Department and our conversation <br /> on June 25, 2001. <br /> Potential or real sources of surface and subsurface contamination caused by past or present land <br /> uses must be documented. My report will include an evaluation of potential non-point sources of <br /> hazardous materials such as on-site and surrounding agrichemical usage and possible point <br /> sources, such as storage tanks and commercial/industrial facilities like the County Dumps. As <br /> noted, there is an apparent nitrate problem under your property and in this general location. I <br /> propose to investigate the origins of the nitrate contamination and discuss prevention and <br /> mitigation measures. However, there is no assurance that the Health Department will not impose <br /> or request nitrate testing on your property and surrounding properties. <br /> This report will be completed on a fixed fee basis f$2.55 . wh' h includes the Report Review <br /> Fee of$17?that I will pay at the time of report submi gain, I cannot guarantee that <br /> chemical testing on the soil and/or groundwater will not be necessary, which would be at an <br /> additional, unknown cost. A Retainer Fee of$1,000 is requested before the start of this Report. <br /> After all information gathering is complete, the findings will be compiled into a written report to <br /> be submitted to the Environmental Health Department. The invoice balance will be due at this <br /> time. <br /> It is important to recognize that considerable uncertainty is inherent in conducting these types of <br /> environmental investigations. Unforeseen conditions may significantly affect our ability to draw <br /> conclusions or make recommendations based on the investigative parameters as promulgated in <br /> the San Joaquin County Ordinances. Therefore,the Client agrees to limit all-encompassing <br /> liability to Valley Ag Research, its owners and agents to the cost of providing this Surface and <br /> Subsurface Contamination Report($2,550). <br /> P.O.
