The site map for the building permit for the proposed structures must be drawn to scale and must
show the location of all buildings, driveways, OWTS, OWTS replacement areas, and any water wells and
surface water within 200 feet of the proposed OWTS and replacement areas. The dispersal area for the
OWTS and reserved replacement area cannot be driven over, paved over, or used for any other purpose.
If you have any questions please contact Michael Kith, REHS, Program Coordinator, at (209) 468-3444 or
mkith@sjcehd.com
IiUTAVt,
Michael Kith, REHS
Program Coordinator
c: Terracon Consultants, Inc.
1868 E.
