The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any applicable <br /> permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to commencing <br /> work. All new addenda supersede the previous ones. <br /> 3 Contact Zuna Barker at(209)953-7311 to arrange the following appointments(a 48-hour notice is required for <br /> inspection appointments). <br /> Observe intergity testing of the 87 STP sump after repairs prior to brine addition, per the manufacturer's <br /> installation instructions, brine type verification and brine addition. <br /> Verify installation of the new 87 STP Bravo test port(installed at the bottom of the sump), per the <br /> manufacturer's installation instructions,verify brine communication inside the 87-STP sump <br /> • Sensor functionality: Functionality of the monitoring system (specifically the 91-octane fill sump sensor,the <br /> 87-octane STP sump brine sensor, 91-octane vent line smart sensor, and the 87 product line smart sensor <br /> located inside the 87-STP sump)must be verified to ensure that monitoring has been restored to meet the <br /> requirements of Title 23 California Code of Regulations and the Health&Safety Code. A copy of the alarm <br /> history shall be printed prior to performing a"cold start"of the monitoring system and made available at the <br /> time of EHD inspection. <br /> Please be aware that the facility cannot operate the 87-octane UST system prior to the EHD inspection and <br /> approval. <br /> 4. If concrete will be broken near the 87-octane tank, a secondary containment test(or equivalent)must be <br /> performed on the 87-octane tank once concrete is poured in accordance with the manufacturer's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, current contractor's <br /> licenses issued by the Contractors State License Board(may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically present at <br /> the work site who possesses a valid, current California UST Service Technician certificate issued <br /> by the International Code Council(ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s)of the component(s) <br /> being tested, repaired, or serviced. <br /> 1868 E.
