REGEIVEP Comments: ATG is currently in alarm for s6 & s7: no vac alarm. OCT `; 20',5 Bellows#10 is damaged. ENVIRONMENTAL Drive off#9. WP:4ITu ncOADTAACAIT Alarms in the past 30 days... Tank 1: overfill 12/29. Tank 2: delivery needed 12/06, 12/13. L2: 87 fill combo piping sump 12/15, 12/20. 12/24. L4: 91 fill combo sump 12/12. 87 PLLD shut down 12/02, 12/11, 12/12, 12/15, 12/20, 12/24. S6: 91 vent vac no vacuum alarm 12/26. S7: diesel product vac no vacuum alarm 12/26. L2 alarms from 12/15 & 12/20 were repaired by Don Lee Pump. L2 alarm from 12/24was repaired by LC Services. L4 was repaired by LC Services. Q 1 alarms were caused by L2 alarms, and were repaired by removing liquid from the sump. (Don Lee & LC Services) S6 & S7 are still in alarm. (see attached work orders) REVISED: I reported an L4 alarm had happened on 12/12, when it actually happened on 12/16. It was a typo, and has been corrected. Items Requiring Follow-Up Actions: Have s6 & s7 alarms addressed ASAP. May want to have bellows#10 replaced. Instructions: Monthly visual inspection of the UST system must be conducted by a Designated UST Operator, who possesses a current"California UST System Operator" certification issued by the International Code Council. A copy of this monthly visual inspection checklist must be provided to the UST Owner or Operator, but not to the State Water Resources Control Board. The Designated UST Operator must alert the UST Owner or Operator of any condition discovered during the monthly visual inspection that may require follow-up actions. The UST Owner or Operator must maintain a copy of this monthly visual inspection checklist and all attachments for the previous 12 months.
