This information was required to be submitted by 5-21-2015. This information has <br /> not been received until a partial RTC was submitted on 8-10-2015 resulting in a non-compliant status for this facility. <br /> An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a <br /> written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br /> inspection report. Provide proof of correction to the EHD. <br /> This is a Class II violation. <br /> 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a Class II violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the September 2014, <br /> December 2014, and March 2015 designated operator monthly inspection reports and failed to check that they were <br /> responded to appropriately. The missing alarms include: S6 alarm on 9-23-2014, L4 alarm on 12-12-2014 (the L4 <br /> alarm was listed as 12-16-2014 instead), S7 alarms from 3-11-2015 and 3-7-2015, and S9 alarms on 3-11-2015 and <br /> 3-6-2015. The alarm follow up actions were available for review at the time of inspection. During the monthly <br /> inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was <br /> documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in <br /> response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at <br /> this facility are including all of the required information on the reports. <br /> This is a Class II violation. <br /> 310 CCR 2636(f)(1) Sensor in a turbine sump failed to activate an audible and visual alarm or stop the flow product. <br /> The S2 and S4 smart sensors failed to activate an audible and visual alarm when tested to ensure continuous <br /> monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or <br /> stop the flow of product at the dispenser when it detects a leak. The sensors were disassembled and inspected then <br /> flipped upside down by the technician onsite, at which time they activated the audible and visual alarm. The above <br /> sensors were then reassembled and tested again (as required) and passed. Note: if this sensor malfunctioning <br /> persists, correct it immediately by having a properly licensed, trained, and certified contractor replace the failed <br /> component with a functional component(LG 113-x listed, if applicable) and obtain a permit within one business day <br /> from the EHD. <br /> This is a Class II violation. <br /> Received by �R Inspector: Phone: Date: <br /> (initial): t , ` ELENA MANZO, Senior REHS (209) 953-7699 08/19/2015 <br /> Page 4 of 6 <br />
