The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. The UDC 3/4 (L-11) sensor was replaced on August 9, 2013 and a permit was not <br /> obtained from the EHD. Emergency repairs relating to UST system monitoring and <br /> release detection may be performed by a properly licensed, trained, and certified <br /> contractor pursuant to the EHD's policy and a permit obtained within one business day <br /> from the EHD. All other repairs must have a permit submitted to and approved by the <br /> EHD prior to beginning work. <br /> 4. Sensor functionality: Functionality of the monitoring system must be verified to ensure <br /> that monitoring has been restored to meet the requirements of Title 23 California Code <br /> of Regulations and the Health & Safety Code. According to the permit application this <br /> sensor was replaced on August 9, 2013; as such sensor functionality was verified on <br /> August 15, 2013, during a routine Underground Storage Tank inspection and Monitoring <br /> System Certification, and no additional inspection is required at this time. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br />
