SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.s4gov.org/ehd <br /> CONTINUATION FORM Page: 2 of 2 <br /> OFFICIAL INSPECTION REPORT Date: 07/14/16 <br /> Facility Address: 1137 W Lathrop, Manteca, CA Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS 11,or MINOR-Notice to Comply) <br /> (Continued from page 1): <br /> be finalized until the revised scope of work is received by EHD. <br /> SUMMARY: <br /> 1) Verified installation of three fill and three turbine sump Bravo Snap and Lock covers (installed on all <br /> onsite tanks). Spill container boots, and boot clamps were also observed on fill sumps. A new sump lid <br /> gasket was installed today on diesel fill sump with inspector onsite. Bravo grease was applied to this <br /> gasket. After adding brine to the L5 sensor reservoir "all functions normal" was observed at the <br /> monitoring panel. <br /> 2) Yellow glue (Bravo type "used to glue sumps", per the technician onsite) instead of fiberglass was <br /> used around the sensor holder in the 87-fill sump- before this permit could be finalized, submit a <br /> statement (supported by appropriate Bravo documentation) indicating that this type of material (the <br /> yellow glue) is approved by the manufacturer for this type of the retrofit installation. Similar yellow glue <br /> was noted around diesel penetration in diesel STP sump. <br /> 3) Violation: At the time of inspection, the following sump sensors were located up the sump saddles and <br /> off the lowest points of their respective sumps, and not located to detect a leak at the earliest <br /> opportunity: diesel fill sump sensor (4-6 inches), 91 fill sump sensor (10-12 inches), and diesel STP <br /> sump (6-8 inches). Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br /> possible opportunity. The technician onsite relocated all above sensors to the lowers points. This <br /> violation was corrected onsite. <br /> 4) Violation: A water mixture with fuel odor (per the technician onsite) was found in the diesel STP sump, <br /> which had the newly installed Bravo Snap and Lock cover. L5 sensor (diesel STP sump brine sensor) <br /> was in alarm at the time of inspection, the technician onsite added brine and "all functions normal" was <br /> observed at the panel. Also, yellow glue (similar to that used in the 87 fill sump) was observed around <br /> the sump penetration. All primary containment must be impervious to the liquid and vapor of the <br /> substance contained. Also, secondary containment shall be impervious to the liquid and vapor of the <br /> substance contained and constructed to prevent structural weakening as a result of contact with any <br /> hazardous substances released from the primary containment. Immediately contact a properly licensed, <br /> trained, and certified contractor to make repairs to the UST system under permit and inspection of the <br /> EHD, or immediately submit a permit for any un-permitted repairs already conducted on this sump. <br /> Submit a corrective action statement to the EHD 30 days from receipt of this inspection report explaining <br /> how each violation was corrected, including any supporting documentation. <br /> As noted earlier, this permit can't be finalized at this time until all appropriate documentation concerning <br /> this permit application has been received by the EHD. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Receiv By: Title: <br /> Elena Manzo (209) 953-7699 �'(� USc_ `rec <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
