As of January 1, 2013, all businesses were required to submit all business plans electronically. <br /> Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to <br /> the EHD for approval. <br /> This is a Class II violation. <br /> 4 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. <br /> 1. The Business Activities and Business Owner/Operator Identification forms have not been submitted for 2019. <br /> This information must be current and submitted as part of the business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and <br /> submit to the EHD for approval. <br /> 2. The Business Owner/Operator Identification form on record was not accurate. The Environmental Contact, <br /> Joseph Lipka has not worked for this facility for greater than 30 days (nearly one year). This information must be <br /> current and submitted as part of the business plan. Immediately log into the California Environmental Reporting <br /> System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for <br /> approval. <br /> This is a Class II violation. <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> 1. Two 55 gallon drums of slam degreaser were found on site that have not been reported. Any material that meets <br /> or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous <br /> material inventory, and submit to the EHD for approval. <br /> 2. The Hazardous Materials Inventory form for the waste oil is not accurate. The maximum size of the largest <br /> container has wrongly been identified as 360 gallons vs. 1,000 gallons and the maximum daily amount is wrongly <br /> been identified as 360 gallons vs 1,360 gallons. This information must be current and submitted as part of the <br /> business plan.
