Leak detectors and annular sensor were missing on site <br /> plan.A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log <br /> into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/and upload a copy of the <br /> UST Monitoring Site Plan. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 10/26/2016 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by 11/25/2016. This <br /> information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br /> inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br /> actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br /> corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. <br /> This is a Class II violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. The facility failed to document <br /> all the alarms and failed to check that they were responded to appropriately. The missing alarms include: <br /> L2: Disp 1-2 Brine(fuel alarm) 10/22/2017, 10/21/2017 and(sensor out alarm) 10/11/2017 <br /> L4: Disp 3-4 Brine(fuel alarm) 10/20/2017 <br /> L6: Disp 5-6 Brine(fuel alarm) 10/22/2017 <br /> L32:vent brine(fuel alarm) 10/22/2017, 10/21/2017 <br /> These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of <br /> all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual <br /> observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other <br /> monitoring equipment,ground water elevations, or other test results; (6)results of inventory readings and <br /> reconciliations.
