Am pm 83333 550 W Valpico Rd, Tracy October 30, 2014 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 317 HSC 25290.1, 25290.2, 25291 Secondary containment not tight. <br /> Less than 1 cup of liquid was found in the diesel fill sump indicating a leak in the secondary containment; the diesel spill <br /> container was tested with passing results at the time of this inspection. A brine fill port was noted in the immediate <br /> vicinity of the observed liquid. Secondary containment shall be impervious to the liquid and vapor of the substance <br /> contained and constructed to prevent structural weakening as a result of contact with any hazardous substances <br /> released from the primary containment. The observed liquid was removed by the technician onsite at the time of the <br /> inspection. Immediately contact a properly licensed, trained, and certified contractor to assess if any repairs to the UST <br /> system are necessary, if necessary, make repairs to the UST system under permit and inspection of the EHD. This is <br /> a minor violation. SUBMIT RETURN TO COMPLIANCE DOCUMENTATION ADDRESSING THIS VIOLATION 30 <br /> DAYS FROM THE RECEIPT OF THIS REVISED INSPECTION REPORT. <br /> 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 87-octane line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of <br /> detecting a 3-gallon per hour leak at 10 psi. The leak detector siphon jet was replaced and leak detector retested <br /> during the inspection. A"UST Retrofit Verification with Inspector Already Onsite"form has been completed and <br /> provided to the operator and contractor. CORRECTED. <br /> 325 CCR 2635(b) Spill container failed to contain a minimum capacity of five gallons. <br /> The 91 octane and 87-octane spill containers failed when tested. All spill containers shall have a minimum capacity of <br /> five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. The 91-octane spill <br /> container gasket was replaced and the spill container was retested during the inspection. A"UST Retrofit Verification <br /> with Inspector Already Onsite" form has been completed and provided to the operator and contractor. A new cap was <br /> installed on the 87-octane spill container and this spill container was also retested during the inspection. CORRECTED. <br /> Overall Inspection Comments: <br /> THIS IS A REVISED REPORT ISSUED ON NOVEMBER 3, 2014 AND IT SUPERSEDES ALL PREVIOUSLY <br /> ISSUED REPORTS. VIOLATION 317 WAS ADDED TO THE ORIGINAL REPORT ISSUED ON OCTOBER 30, <br /> 2014. SUBMIT RETURN TO COMPLIANCE DOCUMENTATION ADDRESSING THIS VIOLATION 30 DAYS <br /> FROM THE RECEIPT OF THIS REVISED INSPECTION REPORT(complete and submit a copy of the Return to <br /> Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or <br /> will be taken for violation 317, and any supporting paperwork). <br /> Consent to perform the inspection, take photos and collect samples was given by Kevin Piper, store manager. <br /> This facility's designated operator is Anthony Abano (ICC expiration date: 5-20-2015 ) . The service technician <br /> was Nikola Zagorov ([CC expiration date: 05-30-2015), who had the following manufacturer's certifications: VR- <br /> 12-13-2014, VMI- 3-18-2016. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if <br /> applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator <br /> Compliance. <br /> Received by(initial): Inspector: Phone: Date: <br /> ELENA MANZO, REHS (209) 953-7699 10/30/2014 <br /> e5of6 <br />
