A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 209 CCR 2715(c)Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the December 2016 <br /> and January 2017 designated operator monthly inspection reports and failed to check that they were responded to <br /> appropriately. The missing alarms include: L-10 sensor out alarm for UDC 11/12 on December 9, 2016 and L6 fuel <br /> alarm for UDC 3/4 on January 18, 2017. During the monthly inspection,the designated operator shall review the <br /> alarm history for the previous month, check that each alarm was documented and responded to appropriately, and <br /> attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> This is a repeat violation, Class II. <br /> 210 CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br /> The May 2016 designated operator monthly inspection reports were not found on site. Designated operator <br /> monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies <br /> of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit <br /> copies to the EHD. <br /> This was corrected on site. <br /> This is a repeat violation, Class Il. <br /> 306 CCR 2630(d)Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The UDC 9/10 sump sensor was raised approximately 2 inches off the lowest point of the sump and not located to <br /> detect a leak at the earliest opportunity. Sensor was located inside a PVC sleeve and sensor was not touching the <br /> bottom of the sump. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br /> opportunity. The sensor was relocated to the lowest point of the sump by the service technician. Ensure that all <br /> monitoring equipment is maintained to detect a leak at the earliest opportunity. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by September 29, 2017. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> FA0002321 PR0231055 SCO01 08/30/2017 <br /> EHD 23-01 Rev.09/22/16 Page 4 of 6 Underground Storage Tank Program OIR <br />
