If a site map element is not applicable for your facility then list it on the map and label as <br /> `NA". Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information,and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 9 HSC 25508(ax1)Failed to annually review and certify that the business plan is complete,accurate,and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System(CERS)annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/,enter the correct or updated information,and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 11 HSC 25505(ax4)Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> There has not been any training on the evacuation and procedures.
