(Cambria) for further investigation at the above <br /> referenced site. The approved work included the installation of two onsite CPT soil <br /> borings and four offsite shallow groundwater monitoring wells. To date, the only work that <br /> has been completed is the onsite CPT borings. <br /> Access agreements and/or encroachment permits have been obtained for three of the <br /> proposed offsite monitoring wells. An access agreement for the fourth offsite well, <br /> proposed monitoring well MW-11, has not been obtained. <br /> PHS/EHD directs the completion of the work plan, minus the installation of proposed <br /> monitoring well MW-11. Because Equiva has been unable to secure an access <br /> agreement with the property owners of the proposed location of monitoring well MW-11, <br /> alternative locations for this well should be investigated. In the meantime, PHS/EHD will <br /> return check #1239 in the amount of$267.00 to Cambria. A new check in the amount of <br /> $178.00 must be submitted to PHS/EHD as soon as possible. Upon receipt of the check, <br /> PHS/EHD will issue the two well permits required for installation of the remaining three <br /> offsite monitoring wells.
