Please add the new federal <br /> hazard categories for this site. <br /> This information must be current and submitted as part of the business plan. <br /> This violation has been corrected. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The site map was not complete as part of the business plan. The site map is missing exits, evacuation staging are, <br /> loading areas, a north orientation, utilities shut offs, and emergency response equipment(fire extinguisher). <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as"NA". <br /> This violation was corrected on site. <br /> This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually.The business only submitted the facility and inventory section on 2/28/2013 that is not <br /> complete and was late.The business made another submittal on 1/14/2015 but only submitted the facility section. <br /> The business made a submittal on 11/21/2019 for 2019.This submittal had all three parts submitted. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year. <br /> This violation was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> All violations were corrected on site. Areas that were inspected: the control room, pump room, diesel tank, and <br /> the sodium hypo chloride tank areas were inspected. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
