1 <br />' West was responsible for overall site management and submittal of <br /> documentation The laboratories used by ERM-West were Sequoia <br /> Analytical of Concord and Superior Analytical of Martinez Both <br /> laboratories are certified by DTSC in the State of California. <br />' A site-specific Health and Safety Plan was prepared by ERM-West <br /> (Appendix J) Potential hazards addressed in the plan included <br /> underground utilities, heavy construction equipment (drill rigs, <br /> concrete saws, and front-end loaders), motor traffic, heat stress, and <br /> potential exposure to petroleum product and/or vapor. Included in <br /> the plan were the local emergency and non-emergency telephone <br /> numbers for the police, hospitals, and fire department, as well as a <br />' map depicting the locations of the nearest hospitals. <br /> ERM-West utilized the services of USA to notify the local utilities to <br /> mark buried utility lines in the vicinity of the tank site. A telephone <br /> line was marked in the vicinity of the former tank location. ERM- <br /> West also utilized Spectrum Exploration to ensure that well bonngs <br /> were placed in locations that were sufficiently removed from <br /> underground hazards (pipes, electric lines, etc.). A copy of <br /> Spectrum's memo of findings is included in Appendix K. <br />' One well permit was required for the four monitoring wells (labelled <br /> MW-1A through MW-4A). Receipt of a permit from EHD was <br /> contingent upon the submittal and approval of an Investigation <br /> Workplan and associated Health and Safety Plan. Copies of the <br /> permit are contained in Appendix L. <br />' In order to install well MW-4A in sods beneath the warehouse floor, <br /> Spectrum utilized the services of a concrete coring company to cut <br /> through the concrete The concrete measured approximately <br />' 6 inches in thickness. <br /> Stockpiled soil situated above the former tank location was relocated <br />' with a front-end loader by Jim Dobbas, Inc. The soil had to be moved <br /> so that monitoring well MW-2A could be installed at the former tank <br /> location. The soil was moved to the front of the east end of the loading <br />' dock and was covered with visqueen. <br />' n <br /> The locations of the four ground water morutaring wells constructed <br /> at the site during the Phase I investigation are illustrated on <br />' Figure 5-2. Well MW-2A was located at the approximate center of the <br /> former tank location. Wells MW-1A and 1VIW-3A were installed to the <br /> north and east of the former tank location, respectively. MW-4A was <br />' installed inside the warehouse building to the south of the former <br /> tank location. The wells surrounding the former tank location were <br /> 54 <br />
