(Apex) for the above referenced site. <br /> In October 1993 Monitoring Well 1 (MW1) had floating liquid hydrocarbons in it, but in January 1994 <br /> floating liquid hydrocarbons were not present in the well, so a water sample was collected. In letters dated <br /> May 6, 1993 and August 20, 1993,PHS/EHD requested that removal and disposal of floating hydrocarbons <br /> be discussed in quarterly reports. There has been no discussion of this in any of the quarterly reports <br /> submitted to date. In the next quarterly report, identify if any petroleum product has ever been removed <br /> from MWl and its disposition. In any future reports,if petroleum product is evidenced in MWl,its removal <br /> and disposal are to be discussed. <br /> In a letter dated February 4, 1994 to Central Valley Regional Water Quality Control Board,Apex states that <br /> they have been authorized by Moore Petroleum "to reduce the frequency of monitoring from quarterly to <br /> semiannually". Please be advised that PHS/EHD is the lead agency providing oversight for the investigation <br /> and remediation of the contamination at this site. PHS/EHD does not concur with reducing the frequency <br /> of sampling the monitoring wells at this site, and is directing you to continue quarterly sampling and <br /> reporting. <br /> Apex further stated in the February 4, 1994 letter that the site is scheduled for underground storage tank <br /> (UST) replacement in early 1995, and at that time over-excavation and groundwater vacuum extraction is <br /> recommended. PHS/EHD agrees that.this is a viable method of remediating contaminated soil and <br /> groundwater. <br /> In June 1992, PHS/EHD concurred with quarterly sampling of the monitoring wells at that time without <br /> active remediation. Based on groundwater sample results since that time, the level of contamination has <br /> remained the same. It appears that active remediation is needed at this site. <br /> PHS/EHD is directing you to submit a time table by April 1, 1994 indicating the dates for: <br /> a) submittal of a closure plan to the PHS/EHD UST unit for removal of the UST's. <br /> b) removal of the UST's. <br /> A Division of San Joaquin County Health Care Services <br /> v <br />
